Draft Notice — Attorney Review Required
This document is a draft and requires attorney review.
Scope of This Website Policy
This Privacy Policy explains how Inland Family Optometry collects, uses, discloses, and protects information through this website. It does not replace our HIPAA Notice of Privacy Practices, which describes how we may use and disclose protected health information and explains patient rights under HIPAA.
Information submitted through this website may become part of a patient record when appropriate. If that occurs, our HIPAA Notice of Privacy Practices also applies.
Information We Collect
We collect information you choose to provide through appointment, contact, quiz, newsletter, contact-lens reorder, and other website forms. Depending on the form, this may include your name, email address, telephone number, preferred office, appointment preferences, reason for contacting us, responses, and the contents of your message.
Our web server and, only after analytics consent, analytics providers may automatically receive technical information such as IP address, browser and device type, referring page, pages visited, and approximate location derived from an IP address.
Please do not use a general website form for an emergency. Website messages are not continuously monitored.
How We Use and Disclose Information
We use website information to respond to requests, arrange appointments, provide requested practice information, process reorder inquiries, administer the website, protect its security, comply with law, and, when you request it, send practice communications.
We may disclose information to service providers that help us host, secure, maintain, and operate the website or process communications. They may use information only to perform services for us, subject to applicable contractual and legal obligations. We may also disclose information when required by law, to protect rights or safety, or in connection with a business transaction as permitted by law.
We do not sell or rent personal information. We do not sell or share personal information for cross-context behavioral advertising as those terms are defined by the California Consumer Privacy Act, as amended by the California Privacy Rights Act (CCPA/CPRA).
Cookies, Analytics, and Consent
The website uses necessary browser storage to remember privacy choices and support core functions. With your consent, it may also load Google Tag Manager and optional analytics services that use cookies or similar technologies to measure visits and interactions.
Nonessential analytics do not load unless you choose Accept or enable analytics in Preferences. You may reject analytics without losing access to the website. Your selection is stored in your browser so that we do not ask on every page view. Clearing browser storage may reset the selection.
You can change a stored choice by clearing this site's browser data and selecting a new preference when the banner appears.
Third-Party Links and Embeds
Pages may contain links to or embedded content from third parties, such as maps, social networks, scheduling tools, or video providers. A third party may receive your IP address and other technical information when its content loads, and its own privacy policy governs its practices. We do not control third-party websites.
California Privacy Rights
Subject to applicability and legal exceptions, California residents may request to know the categories and specific pieces of personal information collected, the sources and purposes of collection, and the categories of recipients; request deletion or correction; and limit certain uses or disclosures of sensitive personal information. California residents also have the right not to receive discriminatory treatment for exercising applicable privacy rights.
Because we do not sell or share personal information for cross-context behavioral advertising, there is no sale or sharing from which to opt out. If our practices change, we will provide the notices and choices required by law.
To submit a data request, call (909) 345-6100 or (909) 345-9809, or write to either office address listed below. We may need to verify your identity and authority before completing a request. An authorized agent may submit a request where permitted by law.
Communications and Retention
You may opt out of marketing email by using the unsubscribe method in a message and may opt out of text messages by replying STOP. Transactional or care-related communications may still be sent as permitted by law.
We retain website information for as long as reasonably necessary for the purposes described here, to meet legal and operational requirements, resolve disputes, and enforce agreements. Retention periods vary by the type and purpose of the information.
Security and Children's Privacy
We use reasonable administrative, technical, and physical safeguards designed to protect information. No internet transmission or storage system can be guaranteed to be completely secure.
This website is intended for a general audience and is not directed to children under 13. A parent or legal guardian should submit requests concerning a minor.
